Bridge To Wellness Project Award Program

The Journey Fund, a Washington non-profit corporation tax-exempt under Section 501(c)(3) of the Internal Revenue Code, assists those affected by cancer who face social, financial, or emotional obstacles on their courageous journey with dignity. By addressing barriers through acts of kindness and dignity, The Journey Fund ensures that no one should feel alone on this journey.

The Journey Fund’s Bridge To Wellness Award Program is a one-time award of up to $25,000.00, as determined by The Journey Fund in its discretion made available by The Journey Fund to Qualified Cancer Patients to help with expenses during their treatment, including:

 

Necessities:

  • Food
  • Housing
  • Lodging
  • Utilities
  • Clothing
  • Transportation

Medical Care Expenses:

  • Durable medical equipment
  • Ostomy/Catheter supplies
  • Wound supplies
  • In-home caregiver (pay for caregiver or help pay for family to relocate to provide care)
  • Vision
  • Dental
  • Complementary therapies

Mental Health Services:

  • Counseling
  • Psychiatry

Other:

Eligibility

To be eligible as Qualified Cancer Patient for this Program, a patient-applicant must:

 

  1. Be a resident of South Puget Sound or be receiving treatment in a facility located in South Puget Sound.
  1. Have a confirmed diagnosis of stage 0-3 cancer.
  1. Authorize the disclosure of protected health information (PHI) and medical records and information to The Journey Fund.
  1. Be receiving active treatment with curative intent.
  1. Have no income.
  1. Have NOT participated in the Program previously as a recipient of a Grant.
  1. Not be an employee, officer, or director of The Journey Fund, or a family member of any of the foregoing individuals.
  1. Agree to all terms and conditions of the Program, as the same may be altered or amended from time to time.

Ineligibility

 

A Patient-Applicant will not be eligible for an Award if the Patient-Applicant fails to meet any of the qualifications set forth above.

Terms and Conditions:

  1. The Patient-Applicant must apply using the form prescribed by The Journey Fund, available at the following URL: www.journeyfund.org.
  1. A Patient-Applicant is limited to receiving one (1) Award. The Patient-Applicant shall be informed of The Journey Fund’s decision via a Notice of Rejection or Award letter.
  1. If approved as a Qualified Cancer Patient, the amount of the Award received by the Patient-Applicant shall not exceed $25,000.
  1. Diagnosis, treatment status, and intent will be verified with the patient’s oncology team once the Patent-Applicant’s application has been received.
  1. The Award is provided on a first come, first served basis and Qualified Cancer Patients who apply are not guaranteed an Award.
  1. Assistance is based funding available to The Journey Fund, and the Program may be suspended, amended, revised or discontinued at any time, from time to time, without notice.
  1. The Journey Fund reserves the right to request additional information from a Patient-Applicant to verify compliance with Program eligibility guidelines, including reporting of use of the Award towards Eligible Expenses. If at any time The Journey Fund determines that any information provided by a Patient-Applicant is untrue, materially misleading, fraudulent, or has otherwise been provided under false pretense, Patient-Applicant’s eligibility for assistance will be terminated.
  1. Unless noted in an applicable Award letter, The Journey Fund will not be responsible for any expenditure made by a Patient-Applicant before funds are committed (i.e., the date the Award letter is issued). Expenditures must be consistent with the list of Eligible Expenses contemplated above. The Journey Fund is not responsible for unallowable expenditures, expenditures that are inconsistent with the Eligible Expenses or Program, or expenditures that exceed the total funded amount of the Award.
  1. The Journey Fund, with reasonable notice to the Patient-Applicant and accommodating the Patient-Applicant’s scheduling conflicts, may require additional reporting or require the Patient-Applicant to participate in site visits, interviews, telephone conferences, presentations or other speaking engagements.
  1. The Journey Fund welcomes and serves all, regardless of race, nationality, religion, faith, gender, sexual orientation, cultural background, and ability.

Survivorship: An emergency application will survive the death of an applicant if:

  1. The application is pending at the time of death;
  2. The application would meet the Emergency Application Criteria if the applicant was alive; and
  3. A surviving spouse, partner, child, or next of kin has confirmed that the need for emergency assistance still exists at the time the application is considered. 

Applications that meet this criteria will be considered by the Application Committee on a case-by-case basis.

*For family units with more than 8 members, add $13,450 annually ($1,121/ month) for each additional member to meet the 250% of federal poverty guidelines.
